Depending on the services that you require, your initial appointment may consist of a consultation explaining your diagnosis and your various treatment options. Routine surgery can usually be performed on the same day as the initial visit. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your first visit:
- Your surgical referral slip from your dentist and any X-rays, if applicable.
- A list of medications you are presently taking including any vitamins or natural dietary supplements.
- If you have medical or dental insurance, bring the I.D. card or a listing of the insurance company name, claim office address and telephone number as well as your personal I.D. number and your group number if applicable.
This information will save time and allow us to submit a claim to your insurance for you. |
IMPORTANT: A parent or legal guardian must accompany all patients under the age of 18 at the consultation visit.
If you are planning to have I.V. sedation or general anesthesia for surgery, please do not eat or drink for six hours prior to your surgery. You will also need an adult to come to our office with you to stay in the office during your procedure, accompany you in our recovery area to receive post-op instructions with you and drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. pregnancy, asthma, diabetes, high blood pressure, artificial heart valves, artificial joints and rheumatic fever, or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
Please Note: Parents and legal guardians are invited to accompany their minor child into the surgical suite at the time of surgery. They are allowed to stay in the room until the anesthesia is administered. Once the procedure is about to begin, all parents and visitors must leave the surgical suite and return to the waiting area until the surgery is complete. After surgery, parents will be directed to our recovery suite to be with their child and to receive post-operative care instructions. |
X-Rays
If your dentist or physician has x-rays that are less than one year old, you may request that they forward them to our office. If there is not enough time for mailing, please pick them up and bring them with you to your appointment. If additional films are necessary, we will be happy to take them at the time of your visit.
