First Visit

front desk staff personThank you for choosing Central Mass Oral Surgery. We want your visit with us to be pleasant one. Please help us to prepare to welcome you as quickly and efficiently as possible by completing the patient registration process online at least one day before your appointment. Simply follow the online patient registration link on our homepage to create your patient record. Your information will be submitted to us over a secure connection which will allow us to prepare your chart and verify your insurance benefits prior to your arrival. If you are unable to submit the record you can print it and bring it with you on the day of your appointment.

If you need assistance, have questions or would like to register by phone, please call our office at 978-534-8300 or 978-632-7270 and our staff will be happy to complete the registration process with you over the phone.

Depending on the services that you require, your initial appointment may consist of a consultation explaining your diagnosis and your various treatment options. Routine surgery can usually be performed on the same day as the initial visit. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your first visit:

  • Your surgical referral slip from your dentist and X-rays, if they were taken recently and are accurately labeled and of good quality
  • A photo I.D. such as a driver’s license or student I.D.
  • A list of medications you are presently taking including any vitamins or natural dietary supplements.
  • If you have medical or dental insurance, please bring the I.D. card or a listing of the insurance company name, claim office address and telephone number as well as your personal I.D. number and your group number if applicable.

This information will save time and allow us to submit a claim to your insurance for you.

If you are planning to have sedation or General Anesthesia

  • It is essential that you DO NOT EAT ANY SOLID FOOD OR HAVE ANY DAIRY PRODUCTS FOR 6 hours before your scheduled appointment. You may continue to drink clear liquids up to 2 hours before your procedure. Clear liquids include water, coffee, tea, carbonated beverages and juices without pulp. You may not drink milk or have any cream in your coffee.
  • A responsible adult must accompany the patient to the office, remain in the office during the procedure, and drive the patient home.  We cannot begin general anesthesia if your ride is not here.
  • The patient should not drive a vehicle or operate any machinery for 24 hours following the anesthesia experience.

Any patient under the age of 18 must be accompanied by a parent or legal guardian.

Instructions for ALL Patients

  • Please wear comfortable loose fitting clothing with a loose neckline, sleeves which can be rolled up past the elbow. Please wear well fitting low-heeled shoes, no sandals, clogs, high heels etc.
  • Please remove contact lenses before your surgery.
  • Please remove all tongue rings/bars.
  • Please bring a current list of all prescription and over the counter medications.
  • If you have an illness such as a cold, sore throat, stomach or bowel upset, please notify the office.
  • If you own an inhaler, please bring it with you on the day of your surgery.
  • If you take routine oral medications, please take all of your medications according to your normal schedule, with a small amount of water on the day of your surgery.
  • PARENTS: Please be advised that NO ONE can stay in the room with the patient during surgery. This policy insures that our patients will receive the best possible care and undivided attention of our doctors and staff.

Any patient under the age of 18 must be accompanied by a parent or legal guardian.
